this is a medical eval center nice place you sign in go see the doctor for recommendation with any medical documents if dont have any you will have to see a primary care dr to get a check up $150 get the letter same day, and of to the store, they have call in 24hr verifications and keep med records on file, on the 4th floor, great staff and understanding doc.
